This form is not a contract to begin lessons.

It is informational only, and will allow me to work with you in scheduling lessons for your child.

Classes are held one day a week, Monday through Thursday, and begin every 1/2 hour between 9:00 A.M. and 6:30 P.M.)

Please be specific and be sure to take travel time from work or school into consideration when making your choices.

Try to be as flexible as possible. You may state preferences, but include other possible times as well. You will not be

contacted if a class becomes available which is outside of your stated time needs.

IMPORTANT:

If ALL of the above information is not provided, your child will not be considered for classes.

Once it is received, I will enter it on our waiting list until a class can be formed which meets your needs.

Due to high demand and low turnover, some time slots are not available. The more flexible you can be, the better your chances of finding a class. Late afternoon classes are particularly difficult to get in to. Though I am occasionally able to place a class during those times, my greatest availability is usually before 3:00 P.M.

I need at least 4 children to start a new class. Ages need to be within 3 months for 2 year olds....within 4 months for 3 year olds....within 6 months for 4 and 5 year olds...and within 1 year for 6 year olds and older.

If you have a group of friends with children of similar age, I can start a new class, assuming all time needs and my availability correspond. This can be a good way to get into the program quickly.

Please be patient, and keep me updated on any changes in your schedule. Occasionally a class will open up on the spur of the moment. I will notify those parents whose provided information matches the opening.

I will make every effort to get your child in as soon as possible. Don't hesitate to contact me periodically to see how things are going. This helps me to know that you are still interested in enrolling your child, and keeps him at the top of the list should a suitable class come available. Thank you for your interest in my program!
